As a permanent part-time Nurse Practitioner within the Interprofessional Primary Care Team, you will utilize your Master's in Nursing and extensive clinical experience. You'll assess, diagnose, and treat patients while managing chronic illnesses and promoting wellness across the community. Your collaborative skills will play a crucial role in enhancing patient care and community health initiatives.

Key Responsibilities:

• Conduct assessments and develop individualized care plans

• Diagnose and prescribe treatments based on evidence-based practices

• Facilitate specialist referrals and interdisciplinary consultations

• Support chronic disease management and health promotion initiatives

• Participate in quality improvement and evaluation of services

Requirements:

• Master of Nursing and NP field diploma required

• Current registration with the College of Nurses of Ontario

• 3 years of recent nursing experience preferred

• Strong communication and problem-solving skills

• Proven ability to work within an inter-professional team
