DescriptionThe Palliative Care Social Worker is an integral member of the Palliative Care Interdisciplinary Team which delivers patient- and family-centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize the quality of life and align the individual's medical care with what matters most to them. Key responsibilities of the Palliative Care SW include evaluating the psychosocial needs of patients and families, providing targeted counseling including anticipatory grief support, facilitating family meetings and goal-setting discussions, making referrals to community resources, and engaging in effective interdisciplinary team collaboration.
This role will work primarily in the outpatient clinic in Scarborough and may also meet patient and family needs at the hospital or in other specialty practice settings through palliative care outreach (in person and/or via telehealth).
This is a position for the professional social worker that involves the evaluation of psychosocial needs of patients/clients and their families, and provision of diagnostic/preventive and treatment services including individual, family, and group therapy. Also provides complex discharge planning, coordinates aftercare services, and makes referrals for other community services based on the treatment plan. The employee works in collaboration with other health/mental health professionals and provides consultation to the treatment team regarding patient/family social and environmental needs.
Work is performed under general supervision in accordance with institutional and social work/clinical counselor policies. May supervise other social workers, students, or other staff.
- Education:
Master's Degree in Social Work/Clinical Counseling from an institution accredited by the Council on Social Work/Counselor Education required. - License/
Certifications:
Current applicable state(s) license as an LCSW (Licensed Clinical Social Worker) or LMSW-CC (Licensed Master Social Worker, Conditional Clinical) required. - Experience:
Two years of relevant experience in a health or mental health setting preferred. Demonstrates competency in all essential functions of a Social Worker I role.
