Support advanced care planning, including discussions around goals of care and end-of-life planning where appropriate.
Contribute to service development, audit, and education to ensure high standards of care and continuous improvement.
We are looking for an enthusiastic, proactive ACP who is passionate about improving outcomes for older adults living with frailty.
