Licensed Practical Nurse Residential Care

Overview

We are hiring a Full-Time Licensed Practical Nurse (LPN) to work in a residential care setting, supporting adults with intellectual and developmental disabilities (I/DD) who require assistance and supervision to live safely and successfully in the community.

Schedule: Monday–Friday, 8am-4pm or 9am-5pm (no nights, weekends, or holidays)

Essential Duties and Responsibilities:

• Administer prescribed medications and treatments per Medication Administration Program (MAP) Guidelines.

• Monitor individuals’ health status, record observations, and report significant changes to the supervisor or physician.

• Take and record vital signs; respond to medical or emergency situations per nursing protocols.

• Support individuals with personal care needs such as hygiene, meals, mobility, and exercise.

• Ensure comfort, safety, and well-being of residents in a residential care setting.

• Act as a liaison between DDS, day programs, and families regarding medical care and support.

• Complete and update medical assessments, monthly progress notes, and other required documentation.

• Conduct medical-based training and guidance for residential staff.

• Assist and collaborate with team members to ensure quality care and program compliance.

Requirements:

• Current LPN license in Massachusetts

• At least 1 year of treating nursing experience

• U.S. citizen or green card holder (no exceptions)
