Medical Social Worker Hospice

Full‑Time Hospice Social Worker / MSW

Location:

Scottdale, Greensburg, and Connellsville (Westmoreland, Fayette), PA.

Base pay: $45,000 – $53,000 per year.

• Provide social work services in accordance with the physician’s plan of treatment.
• Work with the interdisciplinary team directly, or through coordination, to provide assessment and support to patients and families.
• Assess normal and complicated grief and provide bereavement counseling as appropriate.
• Serve as liaison with other community resources.

• Master’s or Bachelor’s level social work degree from an accredited school of social work.
• Licensed to practice as a social worker in the state of PA.
• Minimum one year of hospice experience preferred.
• Valid driver’s license.
