Do you want to make a difference in people’s lives? The DHHS is looking for an outstanding Service Coordinator to serve as a case manager for individuals determined eligible for a Waiver within the Home and Community-Based Services (HCBS) waiver program.
Responsibilities- Coordinate and oversee the delivery of services
- Conduct assessments and eligibility determinations
- Develop plans of services and supports
- Provide appropriate referrals
- Monitor provision of services
- Ensure health, welfare, and safety of those we support
Minimum Qualifications: Bachelor's degree and professional experience in education, psychology, social work, sociology, human services, or a related field (or equivalent experience and coursework, year-for-year). Consideration will be given to applicants who will graduate within 90 days of application and experience in services or programs for persons with intellectual or other developmental disabilities.
Preferred Qualifications: Experience with developing person-centered plans, completing assessments, habilitation, program implementation, team and meeting facilitation, proficiency in Word, Excel, Outlook, Nfocus and Therap. Experience in services or programs for people with special healthcare needs.
Other Requirements: Valid driver’s license and immediate access to a reliable vehicle. State vehicle may be provided dependent upon office location.
