Overview

Join our compassionate team at Cloverdale Rehabilitation and Nursing Center as a Licensed Practical Nurse (LPN). Located at 412 West Cloverdale Rd., Scottsboro, Cloverdale Rehabilitation and Nursing Center is dedicated to providing high-quality care and support to our residents. We are currently seeking skilled and caring LPNs to join our healthcare team.

• Administer medications and treatments as prescribed by physicians, monitor residents for adverse reactions, and provide education on medication management and side effects.
• Assist with resident assessments, including collecting vital signs, monitoring symptoms, and documenting changes in condition.
• Provide direct nursing care, including wound care, catheter care, and assistance with activities of daily living.
• Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement individualized care plans for residents.
• Supervise and delegate tasks to certified nursing assistants (CNAs) to ensure the delivery of safe and effective care.
• Document nursing assessments, care plans, interventions, and outcomes accurately and timely in accordance with regulatory standards and facility policies.
• Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives to enhance resident care and outcomes.
