Hospice Social Worker (Part time) – Glendale, Sun City, Buckeye

Part‑time hours: 20–24 hours per week.

Territory:
Glendale, Sun City, Buckeye, Phoenix.

Provide coordinated care in patients’ homes, across all age groups. Conduct psychosocial assessments, analysis, counseling, and referrals to meet patient and family needs.

• Conduct psychosocial assessments and develop care plans for home‑bound patients.
• Provide counseling and support to patients and families.
• Refer patients to community resources and coordinate follow‑up services.
• Collaborate with interdisciplinary hospice teams to ensure comprehensive care.
• Maintain accurate, timely documentation in the electronic health record.

• Bachelor’s or master’s degree in social work from a state‑accredited program. If hired with a bachelor’s degree after 12/02/2008, must be supervised by a Master of Social Work.
• One year of social work experience in a healthcare agency.
• Knowledge of community resources and strong interpersonal skills. Understanding of hepatitis profile handling.
• Current state certification as a social worker.
• Reliable transportation, valid driver’s license, and current auto liability insurance.
