The Medical Records File Room Clerk position is a service-oriented role that requires a detail-oriented person with a professional and positive attitude. Responsibilities include managing and processing patient-related activities such as electronic documents arriving at fax lines, processing scheduled online exams, radiology films, CDs, and reports. The Clerk will also handle incoming phone calls and work in a collaborative, team environment with internal SMIL departments and external customers.

- Demonstrates a commitment to “World Class Customer Service” and promotes a positive work environment.
- Adheres to the SMIL Attaching Protocol by responding to inbound faxed documents via specified fax lines and using RIS Document Worklist.
- Use appropriate patient search criteria to locate patient records and attach documents.
- Attach and split multiple-page and multiple-patient document faxes.
- Process all orders received electronically via Synapse RIS.
- Adheres to STAT Request Protocol.
- Provide initial contact with patients based on orders from referring physicians and ensure orders are contacted within 24–48 hours with follow-up as needed.
- Complete data entry for online scheduling via SMIL’s Appointment Avenue tool and perform minimal scheduling and order entry in Fuji Synapse RIS and the Appointment Avenue online scheduling tool.
- Set up transportation services for patients’ future appointments, as needed.
- Identify and report issues regarding document management and status activity to the Department Supervisor and/or Contact Center Coordinators.
- Process Outside Read Imaging and Outside Film from other Imaging Companies.
- Answer a high volume of phone calls to CFR for record requests from patients, referring physicians, and internal staff.
- Prepare and process requests for medical records; review records for completeness, assemble into standard order, and file as requested.
- Assemble, sign out, and deliver medical records requested by patients, referring physicians, and internal departments; operate computer to enter and retrieve data.
- Assist other workers with Medical Record tasks; scan materials or copy records to maintain patient files; select and package files for movement, transfer, and permanent storage.
- Transfer outside films/CDs and reports accurately into SMIL’s PACS/RIS systems.
- Same as responsibilities listed above; includes maintaining accuracy and confidentiality in line with HIPAA guidelines.
- Recognizes and performs duties with a high degree of accuracy.
- Skilled in organizing, filing, and retrieving records (electronic and paper formats).
- Effective use of reporting to keep management informed; applies sound analytical thinking.
- Cooperates effectively and provides backup and support; capable of sustaining a high level of concentration and focus.
- Strong written and verbal communication; ability to compose memorandums, emails, and letters with proper grammar.
- Demonstrates world-class customer service; adheres to HIPAA guidelines and maintains confidentiality.
- Maintains a pleasant disposition and professional approach under stress; fosters a positive team environment.
- Adapts to change positively; handles multiple tasks with short timelines, prioritizes, and stays accurate. Knowledge of Fuji Synapse RIS is preferred.
- Dependable and flexible; proficient with Email, Instant Messaging, Internet & Intranet; skilled with standard office equipment (scanner, copier, fax).
- Ability to type 35–40 WPM; reads and interprets written information; contributes to team success.
