We are seeking a dedicated HOSPICE Registered Nurse who is looking for a long-term professional home, values autonomy, and thrives in a supportive, team-oriented environment. Haven is seeking a Full-Time RN Hospice Case Manager to join our exceptional team in the WEST Valley Territory :
Including Glendale, Peoria, AZ, Surprise, Good Year, AZ and surrounding areas to provide meaningful home hospice visits.

Position Responsibilities :
- Focus on quality over quantity.
- Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans.
- Educate patients and families on the hospice philosophy, symptom control, and end-of-life care.
- Maintain accurate, timely documentation in compliance with company and regulatory standards.
- No traditional night or weekend on-call!

Qualifications :
- Current Registered Nurse (RN) license in the State of Arizona (required)
- Minimum of 1 year of hospice experience required
- Reliable transportation and valid drivers license (required)
- Compassionate and professional bedside manner
- Strong communication and organizational skills
