Optimal Wound Care | Pennsylvania

Optimal Wound Care is looking for an experienced Nurse Practitioner with a passion for wound care to join our growing mobile wound-care team.

We provide specialized wound care directly to patients in their homes, assisted living communities, rehabilitation centers, and post-acute care settings. Our goal is simple: provide high-quality, specialized wound management where patients need it most.

This is a great opportunity for an NP who enjoys autonomous clinical practice, building relationships with patients and care teams, and treating complex wounds in a mobile setting.

What You'll Do

  • Perform comprehensive wound assessments and develop individualized treatment plans
  • Evaluate and treat chronic, acute, and non-healing wounds
  • Perform wound debridement and other advanced wound-care procedures
  • Manage diabetic foot ulcers, pressure injuries, venous and arterial ulcers, surgical wounds, and sacral wounds
  • Utilize advanced wound-care therapies and grafts when clinically appropriate
  • Monitor wound progression and adjust treatment plans based on patient response
  • Collaborate with physicians, home health nurses, case managers, assisted living teams, and other healthcare professionals
  • Provide education to patients, families, caregivers, and clinical teams
  • Document assessments, treatment plans, procedures, and follow-up care accurately
  • Help identify complications early and support efforts to reduce avoidable hospitalizations

What We're Looking For

  • Current Nurse Practitioner (NP/CRNP) license in applicable states
  • Board certification as a Nurse Practitioner
  • Wound-care experience strongly preferred
  • Experience with chronic, complex, diabetic, pressure, venous, arterial, or post-surgical wounds
  • Experience performing wound debridement preferred
  • Strong clinical assessment and decision-making skills
  • Comfortable working independently in a mobile/community-based environment
  • Excellent communication and documentation skills
  • Ability to collaborate effectively with multidisciplinary care teams
  • Valid driver's license and reliable transportation

Why Join Optimal Wound Care?

  • Mobile clinical practice — provide care where patients live
  • Work with a variety of complex wound cases
  • Greater clinical autonomy and independence
  • Build meaningful relationships with patients and healthcare teams
  • Collaborative environment with other healthcare professionals
  • Opportunity to help improve healing outcomes and reduce complications
  • Growing organization with opportunities to make an impact
