What you will do:
  • Support safe and timely discharges by completing occupational therapy assessments, contributing to multidisciplinary planning, and reducing delays.
  • Undertake short stay reviews to evaluate outcomes, adjust care plans, and ensure equipment and support remain appropriate.
  • Work autonomously in a fast-paced environment, applying clinical reasoning to promote independence, manage risk, and maintain accurate records.
  • Collaborate effectively with colleagues, families, and partner services to deliver top-quality care.
Requirements:
  • Registration with the Health and Care Professions Council (HCPC) is essential.
  • A full driving licence is required as you will need to travel to different locations as part of your role.
