Community Nurse (North Lincolnshire)

Please note - this post is based in North Lincolnshire.

The community nurse will work as part of an integrated team contributing to family health needs assessment, working with families, children and young people 0‑19 years and up to 25 years with SEND. The post holder will work in partnership with families and other agencies to develop, implement and evaluate packages of care on an individual basis and with groups.

The community nurse will contribute to the wider public health agenda for Children and Young People through individual, group and community programmes of care.

Qualifications:

Registered Nurse (RN, RSCN or RNC) with post‑qualification experience.

You will receive regular safeguarding, management and clinical supervision and support from the leadership team.

This role is offered by the Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH), which employs around 4,000 colleagues and provides services across mental health, physical health, learning disability and drug and alcohol services.
