Job Description:
We are seeking a dedicated and compassionate Social Services Coordinator to join a team in a long term care facility in Seaford, DE. As a LTC Social Services Coordinator, you will be responsible for providing social services and support to individuals in need, coordinating community resources, and advocating for clients' rights and well-being.
Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide counseling and support to clients and their families
- Coordinate and collaborate with other healthcare professionals and community agencies
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date documentation of client interactions and progress
Qualifications:
- Bachelor's degree in Social Work or related field
- 1 year of long term care experience
- State licensure or certification as required
- Previous experience in social services or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic approach to client care
