Shift: 8:00 AM - 4:30 PM, Monday through Friday with occasional weekend needs
Pay Rate: $28/hr, OT $42/hr
Contract Duration: 13 Weeks

Job Description:
We are seeking a dedicated and compassionate Social Services Coordinator to join a team in a long term care facility in Seaford, DE. As a LTC Social Services Coordinator, you will be responsible for providing social services and support to individuals in need, coordinating community resources, and advocating for clients' rights and well-being.

Responsibilities:

• Conduct assessments to determine clients' needs and develop individualized care plans
• Provide counseling and support to clients and their families
• Coordinate and collaborate with other healthcare professionals and community agencies
• Advocate for clients' rights and access to services
• Maintain accurate and up-to-date documentation of client interactions and progress

Qualifications:

• Bachelor's degree in Social Work or related field
• 1 year of long term care experience
• State licensure or certification as required
• Previous experience in social services or a related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and empathetic approach to client care
