Key Responsibilities:

• Coordinate and participate in interdisciplinary care conferences

• Provide staff education on best social work practices

Requirements:

• Bachelor/Masters of Social Work or equivalent credentials

• In good standing with the Ontario College of Social Workers

• Experience in gerontology or long-term care settings

• Strong documentation and organizational skills

• Capacity to handle sensitive situations with professionalism
