Physical Therapist – Home Health Services – Seaside, Oregon
We are offering a $7,500 hiring bonus for eligible external hires that meet the required qualifications and conditions for payment.
Schedule: Full time, Day Shift.
We manage and provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.

- Bachelor's Degree From an accredited Physical Therapy Program. Or Master's Degree From an accredited Physical Therapy Program. Or D.P.T. From an accredited Physical Therapy Program.
- Within 30 days of hire:
National Provider BLS – American Heart Association. - Upon hire:
Oregon Physical Therapist License. - 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate’s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
