The Harborview Medical Center – Social Work Department has an outstanding opportunity for a Gun Violence Inpatient Consult Social Worker to join the Violence Intervention and Prevention Program .

WORK SCHEDULE
  • 40 Hours/Week

POSITION HIGHLIGHTS
  • The Violence Intervention and Prevention Program (VIPP) serves diverse, medically, and psychosocially complex patients with trauma related injuries secondary to gun violence
  • Support victims of gunshot violence being seen either in the Emergency Department, Inpatient, or in Specialty Clinics. Care needs for these patients and families can be extensive and complex, requiring coordination across multiple providers, systems, etc. and may include short-term case management work
  • Coordinate tasks that are implemented to successfully transition patients from HMC to their community. This position serves as a liaison for patients and families with medical teams, nursing staff, outside agencies and providers, interdisciplinary team members, consulting and referring physicians and agencies involved in the care of patients from admission through the discharge process
  • In conjunction with the patient and interdisciplinary care team, this position will identify the clinical and financial targets and milestones for recovery and eliminates barriers as needed and communicate the transition plan to appropriate team members
PRIMARY JOB RESPONSIBILITIES
  • The Social Worker will work in a Consult Role to the primary inpatient team, and along side with the VIPP Intervention Prevention Specialist.
  • Provide specialty consultation to other health care staff and health care teams regarding psychosocially focused aspects of client/patient care and gun violence. Provide psychosocial support to patients and families
  • Provide direct social work services to patients and families within assigned service area while adhering to the parameters of HMC policies and procedures and NASW Code of Ethics.
  • Provide care coordination for patients with complicated social and medical histories
  • Provide case management to identified patients for 90 days after discharge
  • Work with patients to develop safety plans after discharge
  • Assess Social Determinants of Health needs and refers patient and families to appropriate resource agencies.
  • Make timely referrals to hospital and community agencies including those required by mandatory reporting laws such and child and vulnerable adult abuse
  • Maintain working knowledge of community resources, processes, interventions and requirements pertaining to assigned service area, including but not limited to:
    • Mental health crisis intervention, including suicide risk assessment
    • Interpersonal violence
    • Abuse/neglect of children and vulnerable adults
    • Crisis intervention
    • Chemical dependency
  • Provide skilled social work services to clinic patients of all ages, including but not limited to:
    • Comprehensive psychosocial assessment
    • Focus on harm reduction and trauma informed care
    • Systems advocacy and intervention
    • Brief interventions
    • Care management across multiple service providers as indicated
    • Grief counseling
    • Crisis intervention
    • Information and referral to appropriate community resources
REQUIRED POSITION QUALIFICATIONS

A Master of Social Work Degree from a program accredited by the Council on Social Work Education

AND

One year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENTS

Must hold one of the following credentials issued by the state of Washington:

  • Licensed Independent Clinical Social Worker

$88,524.00 annual

Other Compensation:

$5,000 Sign-On Bonus Certification Premium - $1.50/hour
