Diabetes Educator - Maternal Fetal Medicine at Providence Health and Services summary:

The Diabetes Educator in Maternal Fetal Medicine provides patient education on diabetes self-management and nutrition, develops and implements personalized educational plans, and supports ongoing patient care coordination. They serve as a resource for patients, communities, and healthcare professionals, and conduct continuing education programs. This role requires clinical nutrition expertise, certification in dietetics and diabetes education, and experience in teaching and public speaking within healthcare settings.

Description

Educates patients about diabetes self-management as well as evidence-based nutrition guidelines in individual or group settings, using approaches that are culturally relevant and age appropriate. Assesses the needs of patient and their family, formulates an educational plan, implements the plan, and evaluates and revises the plan as needed. Functions as a resource to the patient, the community, and other health care professionals both within and outside the Center.

Coordinates ongoing patient care and support with appropriate members of the care team. Provides education, training, and support related to diabetes technologies. Plans and conducts continuing education programs in diabetes and general nutrition for allied professionals and the general public. Participates in the assessment, planning, implementation, and evaluation of the diabetes education program at the Center, including continuous quality improvement and other activities related to maintaining recognition with the American Diabetes Association.

Demonstrates competence in written and oral communication skills.

Required qualifications:

  • Bachelor’s degree in Nutrition Sciences or Dietetics
  • Washington Dietitian Certification upon hire or Washington Nutritionist Certification upon hire
  • National CDR Registered Dietitian - Commission on Dietetic Registration upon hire
  • Certified Diabetes Care and Education Specialist (CDCES) within 1 year of hire
  • 4 years of nutrition experience, preferably one year in ambulatory care setting or community health
  • One thousand hours of diabetes education experience in a variety of settings
  • Experience and comfort with teaching and public speaking

Preferred qualifications:

  • Master's degree in Nutrition Sciences or Dietetics
