Are you a dynamic nursing leader passionate about improving care coordination and patient outcomes? We’re seeking a Director of Care Management to lead a high-performing team focused on quality, collaboration, and compassionate care over 2 thriving acute care hospitals in the Pacific Northwest region.
In this pivotal role, you’ll oversee care management and utilization review functions that ensure every patient receives safe, seamless, and cost-effective care from admission through discharge. You’ll guide a talented interdisciplinary team and partner closely with clinical and operational leaders to elevate the patient experience and optimize performance across the continuum of care.
What You’ll Do:- Lead care coordination, discharge planning, and utilization review processes.
- Drive systemwide initiatives that support clinical excellence and efficient resource use.
- Coach and mentor a skilled team of nurses and care management professionals.
- Collaborate with physicians, community partners, and payors to ensure smooth transitions of care.
- Use data and analytics to track trends, improve outcomes, and support organizational goals.
- Bachelor’s degree in Nursing (Master’s preferred).
- RN with at least 2 years of acute care experience
. - 3+ years in care coordination or utilization review leadership at the Director level.
