Concerto Care is seeking an Advanced Practice Provider – Nurse Practitioner or Physician Assistant to be a core member of a multi‑disciplinary Field Team that delivers home‑and community‑based care to patients with complex needs.

• Deliver clinical care flexibly, whether it be in‑person at your patient’s home or virtually from your own home.
• Provide compassionate, individualized, and holistic patient care including comprehensive primary care, urgent care, and chronic condition management.
• Collaborate with our multidisciplinary care team to cooperatively deliver transitions of care support, social determinants of health interventions, and complex care management.
• Benefit from professional development opportunities via dedicated clinical education programming and active engagement with our on‑staff world‑class experts in geriatric medicine, palliative care, geriatric psychiatry, clinical pharmacy, and complex care management.
• Participate in after‑hours on‑call coverage as needed.

• Nurse Practitioner with graduation from an accredited health professions school and successful completion of an ANCC or AANP certification; or board‑certified Physician Assistant. Nurse Practitioners who only possess PMHNP or NNP certifications are not eligible for this position.
• Active licensure in the state of Washington.
• Active DEA licensure and state‑specific controlled substance registration, as applicable.
• Certification in basic life support (BLS) is required.
• DRIVER REQUIREMENTS IF DRIVING YOUR OWN VEHICLE:
  Licensed for a minimum of 5 years. No vehicle‑related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at‑fault accident in the last 3 years.

Base Salary/Wage Range $145,000 to $165,000 annually plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside the range shown.
