Licensed Practical Nurse – PACE Clinic

Base pay range: $30.25/hr – $44.29/hr

The Licensed Practical Nurse will provide skilled nursing services in the PACE clinic and support the transition of care for PACE participants. Responsibilities include following up on patients post‑hospital discharge, post‑ER visits, post‑procedures, post‑surgery, and monitoring any acute changes or symptoms via patient phone calls. The role seeks to avoid readmissions to hospitals and skilled nursing facilities by managing all aspects of care from inpatient stay to home.

The LPN will identify high‑risk, complex patients and communicate with all entities involved in the care of the patient to promote and maximize care coordination. The position uses all components of the transitional care model and nursing process—including assessment, triage, planning, implementation, and evaluation—to meet patients’ needs, and plans, organizes, and provides nursing services to clinic patients in conjunction with clinic providers and registered nurses.

• Completion of a Licensed Practical Nurse program.

• One (1) year of LPN experience required, along with proficiency in utilizing electronic health records.

• Experience with the geriatric population is preferred.

• Active Washington State LPN license.

• Fluency in English; fluency in any API languages preferred.

Monday to Friday, 8:00 am–5:00 pm.
