LPN Part-Time

• LPN will be supervised by an RN
• Provide safe and effective care for the client based on individualized Plan of Care
• Provide prompt communication of changes in patient medical status or emergencies to the team for effective management of cases
• Provide safe and effective nursing for the patient based on the nursing scope of practice and Plan of Care in conjunction with the Nurse Supervisor/Manager of Clinical Practice. May include medication administration, wound care, infusion, transferring and positioning the patient, catheter care, and effective communication with the client, family, and Team Case Manager
• Demonstrate empathy, caring and self-confidence; attention to detail; anticipate problems and know whether to diffuse or escalate; prioritize and demonstrate a sense of urgency

• Current Nursing license, state of Florida
• Possess wound care skills
• IV certification preferred for LPNs
• Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care
• 1 year of Home Health experience preferred
