HHS require visiting patients in their homes.
Base pay: $120,000 – $135,000 per year, with annual bonuses of $10 000 (Level 1) or $20 000 (Level II) paid quarterly. Pay is commensurate with experience.
Responsibilities- In partnership with other clinicians on the case, evaluate and address the needs of individuals in their homes so they can live safely and productively.
- Perform an evaluation in the home and develop a care plan; work with PTAs to follow the plan.
- Help patients with mobility, movement, and regaining independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure a quality level of care.
- Evaluate and report patients’ progress to MD.
- Restore function to prevent or minimize disability following disease or injury and help the client reach maximum performance within their capabilities.

- All candidates must complete a computer literacy and competency assessment prior to scheduling an in‑person interview.
- Current unrestricted CA Physical Therapist license.
- Current BLS/CPR certification.
- Valid CA driver’s license.
- Knowledge of local community resources.
- Knowledge of individual development and human behavior as it relates to illness/injury and the influence of culture on health care.
- Knowledge of state and federal home‑care regulatory guidelines (preferred).
- Experience in home health care (highly preferred).
- Experience with high acuity patients (plus).
- Experience working with an interdisciplinary team (highly desired).
- Case Management fundamentals (highly desired).
- Self‑directed, empathetic, critical thinker.
- Outstanding interpersonal skills, leadership, and organizational abilities.
