Registered Nurse Case Manager – Home Health

Territory: Sebastopol area

Employment Type: Full‑time •
Seniority Level: Entry level

Salary: $130,000 – $160,000 per year, plus annual bonus and incentive program.

Responsibilities
  • Develop, implement, and execute individualized care plans to promote wellness and prevent re‑hospitalization.
  • Conduct full assessments of patients and families during initial visits.
  • Educate patients and family caregivers on wound care, blood draws, tube feedings, tracheostomy care, Foley catheters, CHF management, and medication administration.
  • Coordinate care with interdisciplinary team members (physicians, PT/OT, social workers).
  • Prepare and maintain clinical notes, orders, and charting in a compliant timeframe.
  • Initiate and administer IV medications (e.g., antibiotics, diuretics) safely and in accordance with protocols.
  • Provide ongoing case management throughout care episodes.
Qualifications
  • Current, unrestricted California RN license.
  • Current BLS/CPR certification.
  • Valid California driver’s license.
  • Experience with high‑acuity home health patients preferred.
  • Background in interdisciplinary team settings and case‑management fundamentals desirable.
  • Proficiency with IV therapy and infusion protocols.
  • Self‑directed, empathetic, critical‑thinking, and strong interpersonal and leadership skills.
