SUBMISSION REQUIREMENTS
- Must have Medical License and DEA at time of Name Clear
- Must have clean malpractice/background for temp privs
- Must have Board Certified EM
- Must send self-query NPDB and EPA with presentation
- Must be able to work at least 7 shift per month
- Must have MM/YYYY format on CV and gaps be explained
- Must have all licenses and certifications listed on CV
- Must have current start date and availability
AVAILABILITY
- 1st possible date they would be available?
- How many shifts would they be able for per month?
- What month is their schedule completely open?
- Any dates in the next 3-6 months they cant work?
- Any Holidays in the next 3-6 months they would be open to work.?

Coverage:
- Main ED 88 hours,
- Peds 24 hours, and
- Mid-Level 59 hours

- Must have FL license and DEA at time of presentation
- Must have clean malpractice/background for temp privs
