Job Description

The Social Worker is a member of the core interdisciplinary team, responsible for assessing psychosocial symptoms of patients and their families, developing outcomes to alleviate these symptoms, and providing psychoeducation, supportive counseling, and casework interventions. The Social Worker also offers psychosocial education and consultation to the Interdisciplinary Group (IDG).

• Participates in preparing, evaluating, and executing the interdisciplinary care plan to ensure high-quality psychosocial care and continuity of services.
• Performs bio-psycho-social-spiritual assessments of patients and families, focusing on support rather than diagnosis.
• Provides individual and family supportive counseling.
• Refers patients and families to appropriate community resources.
• Maintains accurate and timely documentation in the patient’s medical records.
• Attends weekly team meetings for care planning.
• Participates in patient case reviews.
• Provides psychosocial education and consultation to the team.
• Participates in supervision meetings and trainings; may supervise other psychosocial team members, including interns.
• May coordinate and communicate volunteer interventions within the care plan.
• Participates in psychosocial spiritual on-call rotations as assigned.

Job Requirements

• Ability to travel as required.
• Thorough knowledge of community resources or willingness to acquire it.
• Understanding of psychosocial issues in terminally ill patients and their families, including effective intervention strategies.
• Ability to work well within an interdisciplinary team.
• Excellent listening and communication skills.
• Effective advocacy for patients and families within the care team and community agencies.

Qualifications

• Master’s of Social Work from an accredited school (or grandfathered in per 2008 Medicare Hospice Conditions).
• At least one year of social work experience in healthcare.
• Experience with terminally ill patients and families preferred.
• State licensing/certification as required.

CT: LMSW, DC: LGSW, DE: LMSW, GA: MSW supervised by LCSW until LMSW, IL: LSW, NJ: LSW, OH: LSW, TX: LMSW, WI:
Social Worker Certificate.
