Home Health Physical Therapist Assistant

A Physical Therapy Assistant provides skilled education, treatment, and interventions under a physician's plan of care in a home health setting. They collaborate with an interdisciplinary team to optimize patient well-being and ensure safe, effective therapy services.

• Deliver direct physical therapy according to the physical therapist's guidance and physician’s orders.
• Communicate regularly with patients and caregivers about the care plan.
• Provide treatments, procedures, and education to promote pain relief and functional development or restoration.
• Monitor and report patient conditions, reactions, and progress to supervisors and physicians.
• Educate patients and caregivers on device usage, rehabilitation techniques, and safety measures.
• Coordinate with other disciplines on patient care and plan discharge and follow-up instructions.
• Maintain documentation, ensure confidentiality, and comply with ethical standards.
• Participate in in-service training and adapt to changing needs.

• Graduate of an accredited Physical Therapist Assistant program.
• Current, unrestricted state license.
• Valid CPR certification.
• Strong communication, collaboration, and professional conduct skills.
