Sebring, FL. Full‑time, Monday‑Friday 8 am – 5 pm, with daily travel within Highlands and/or Hardee County.

Overview

Compassionate Care Hospice, an Amedisys company, is seeking an RN Case Manager to join our Sebring team of dedicated healthcare professionals. This role focuses on delivering high‑quality hospice and palliative care to terminally ill patients in their homes and in the community.

Salary and Benefits

Salary: $80,000–$84,000 annually, with potential bonus opportunities. Final compensation may vary based on location, knowledge, skills, and experience.

Job Description

As an RN Hospice Case Manager, you will coordinate hospice nursing and palliative care for terminally ill patients within the community. You’ll collaborate closely with physicians, nurses, families, and other members of the care team to ensure excellent pain and symptom management and high‑quality end‑of‑life care.

Typical duties include:

  • Case managing 15‑18 patients.
  • Attending interdisciplinary team meetings and team huddle calls.
  • Assigning aide visits.
  • Coordinating care with social workers, chaplains, volunteers, and bereavement teams.
  • Developing and nurturing relationships with facility staff and community leaders.
  • Utilizing technology such as HCHB, Optum Pharmacy solutions, IPrescribe, Forcura, Medline, Talent

    EDGE, EBSCO, and tablets with voice dictation.
Responsibilities
  • Perform patient assessments and collaborate with the care team to develop and implement care plans.
  • Make referrals to other hospice disciplines or document rationale for not doing so.
  • Maintain accurate and timely patient care progress records.
  • Record pain/symptom management changes and outcomes.
  • Teach patients, families, and caregivers appropriate care techniques.
  • Communicate regularly with clinical manager and care team.
  • Perform on‑call responsibilities and provide on‑call services as assigned.
  • Other duties as assigned.
Qualifications
  • 1+ year of nursing experience in an acute care, ER, or fast‑paced environment.
  • Current RN license in the state(s) of assignment.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and liability insurance.
  • Experience with HCHB and/or prior hospice experience is a huge plus.
Training and Onboarding

The typical orientation and onboarding period is 3–4 weeks, individualized based on experience.
