Role DescriptionThis part‑time hybrid role for a Nurse Practitioner is located in Sebring, FL, with some work‑from‑home acceptable. The Nurse Practitioner will provide primary and chronic care management, coordinate care for patients in ALFs, group homes, and private homes, conduct assessments, diagnose conditions, develop treatment plans, prescribe medications, collaborate with other healthcare providers, perform in‑person visits, maintain patient records, and ensure compliance with healthcare regulations.
Qualifications- Experience in primary care, chronic care management, or transitional care
- Ability to conduct assessments, diagnose conditions, and develop treatment plans
- Proficiency in telehealth services and electronic health records (EHR) systems
- Strong communication, interpersonal, and organizational skills
- Current Nurse Practitioner license in the state of Florida
- Ability to work independently and as part of a team
- Experience in working in ALFs, group homes, or home health settings is a plus
- Master’s Degree in Nursing
- Active Florida license
