As a Nurse Case Manager, your focus will be on coordinating care for clients with long-term illnesses, employing principles of chronic disease management. You will collaborate with various health professionals to ensure clients receive comprehensive care, while acting as an educational resource for your team and patients. Your leadership will help drive client success in their health journeys.
Key Responsibilities:
• Manage chronic care case loads and patient assessments
• Liaise with interdisciplinary teams for effective client plans
• Provide ongoing evaluations and interventions for client care
• Conduct educational workshops for clients and families
• Use clinical expertise to support health and wellness initiatives
Requirements:
• Valid RN or RPN registration with BCCNM in BC
• Three years’ clinical nursing experience required
• Personal vehicle required for local travel
• Proficient in community-based nursing practices
• Strong communication and conflict resolution skills
