This is a full‑time position with an expected 30 productivity points per week. Clinicians are paid per point and may earn additional pay for exceeding 30 points.

Adaptive Home Health is a privately owned agency delivering high‑acuity, patient‑centered care throughout Texas. Our clinicians are supported by an experienced operational team.

• Provide comprehensive care in patients’ homes, including skilled documentation through Homecare Homebase.
• Treat wounds confidently.
• Advocate for patients and coordinate communication with physicians, referral sources, caregivers, and facility management.
• Administer medications and treatments as prescribed, including infusion therapy, wound care, teaching, disease management.
• Participate in regular case conferences and in‑services.
• Serve approximately five patients a day Monday‑Friday.
• Organize and manage time effectively, multitask and meet timelines.
• Maintain a professional appearance and effective communication skills.

• Current Texas LVN license.
• Three years of clinical nursing experience.
• Current Texas driver’s license.
• CPR certification.
• Experience with healthcare software.
• Competency to perform any skilled procedure ordered by a physician within the scope of the Nurse Practice Act.
