Job DescriptionMetro is seeking a motivated and passionate FULL TIME Physical Therapist to provide in‑home care. All levels of experience are encouraged to apply; we will train the right new graduate.

- Accept and treat patients in your designated geographic area
- Evaluate patient needs and develop individualized treatment plans
- Develop plan of care and treatment goals with physician, patient, and family
- Plan, prepare, and implement customized therapy programs
- Instruct family members, PTAs, Home Health Aides, and caregivers on therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
- Physical Therapist Program Graduate
- Licensed Physical Therapist in New York or eligible
- Mandatory certifications:
First Aid, AED, and CPR - Home‑health experience preferred; mentorship provided
- Ability to travel to patients’ homes within assigned geographic territory

The anticipated base salary range is $85,000.00–$. Salary is based on experience, knowledge, skills, and geography. The position is eligible for incentive compensation subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are available subject to company plan terms.
