** JOB RESPONSIBILITIES :*
* • Collaborates with dialysis providers to ensure communication between facility and dialysis centers is consistent, accurate, and information is followed upon  
• Responds to resident concerns regarding dialysis care  
• Monitors the process of obtaining accurate weights of dialysis residents  
• Trains licensed nurses on assessment skills for monitoring dialysis residents and monitors licensed nurse alert charting compliance  
• Ensure staff are competent with dialysis care, vascular access devices, fluid and dietary restrictions, etc  
• Communicate with medical practitioners and other clinical managers to effectively problem solve and manage the facility dialysis population  
• Participate/lead quarterly reviews of resident outcomes, dialysis efficiency, anemia management, and overall dialysis program success
**** QUALIFICATIONS & EXPERIENCE REQUIREMENTS:
***** Registered Nurse in the state of Indiana
* Registered Nurse with at least 5 years medical/surgical experience
* Previous Long Term Care Experience
