- Direct and coordinate all nursing care based on established clinical nursing practice standards.
- Collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.
- Ability to practice nursing as described by the definition: “The diagnosis and treatment of human responses to actual or potential health alterations.” Utilize the nursing process in providing care including administering treatments and medications, interpreting diagnostic tests, formulating a plan of care, collaborating with other health care providers, and educating patients and/or significant others. Pass competencies for this position. Exercise discretion and independent action within prescribed limits.
- Collect patient health data and determine data collection priorities based on the patient’s condition.
- Collect pertinent data using appropriate assessment techniques and involve patients, significant others, and health care providers as appropriate.
- Ensure data collection is systematic and ongoing; document relevant data in the medical record according to hospital standards; analyze assessment data to inform diagnoses and patient problems.
- Derive diagnoses/patient problems from assessment data and validate with the patient, significant others, and health care providers when possible.
- Document diagnoses/patient problems in a manner that facilitates determining expected outcomes and plan; identify patient-specific expected outcomes.
- Derive measurable outcomes and formulate/outcome plans with the patient and health care providers; ensure outcomes are realistic and attainable given resources and patient capabilities; include a time estimate for attainment.
- Develop and individualize a plan of care; collaborate with the patient, significant others, and health care providers; ensure the plan reflects current nursing practice and provides continuity of care.
- Establish and implement interventions consistent with the plan of care; document interventions and evaluate patient progress toward outcomes; ensure ongoing evaluation and revision of diagnoses, outcomes, and the plan of care as needed.
- Involve the patient, significant others, and health care providers in the evaluation process when appropriate; participate in quality-of-care activities to monitor and improve nursing care and patient outcomes.
- Identify aspects of care important for quality monitoring; collect and analyze quality data; formulate recommendations to improve nursing care or patient outcomes; implement changes to improve nursing practice.
- Other duties as assigned.
- Bachelor’s Degree preferred
- Graduate of an accredited school of professional nursing
- Licensure as a Registered Professional Nurse (RN)
- Minimum of one year of hospital clinical experience preferred
