• Conduct interdisciplinary team meetings to determine appropriate services
• Provide accurate, timely, complete, and acceptable documentation
• Write State Plans
• Monitor patient progress and respond to changes in condition
• Assist in family planning consultations and education
• Assist with the transition of the individual from services, if necessary, and planning for any future needs
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Provide education and support to patients and their families regarding health management
• Maintain accurate patient records and documentation in compliance with healthcare regulations
• Participate in quality improvement initiatives and adhere to safety protocols

• Valid Registered Nurse (RN) license
• Experience in Home Health Care preferred
• Strong interpersonal skills for effective communication with patients, families, and team members
• Commitment to ongoing professional development and staying current with nursing practices
• Home Health: 1 year experience preferred
• License: RN License required
• Ability to commute: required
