CASE MANAGER WANTED in Sequim, WA. 40 hrs./wk. Non-Exempt. Salary range $68,401 - $89,749 annually.
Benefits include: 12 paid holidays per year, paid annual and sick leave accrual, WA PERS pension plan, medical plan including vision, dental plan, EAP counseling, life and AD&D insurance.
Provides case management for in-home care to seniors and adults with disabilities including assessment of care needs, creating service plans, coordinating care providers; focusing on client independence.
BA in behavioral or health sciences and 2 yrs. paid social service experience; or BA and 4 yrs. paid social service experience; or Master's Degree in behavioral or health sciences & 1 yr. paid social service experience.
