Position Summary

The Care Team Clinical Manager oversees the clinical care for a localized population of patients and provides direct management of the clinical members of that care team. Core functions are to enhance the performance of the entire team through facilitating the work of the various members in the areas of clinical performance and quality, patient satisfaction, compliance with Eventus clinical protocols, processes and procedures, and achievement of productivity expectations as measured by patients receiving needed services.

ESSENTIAL DUTIES AND RESPONSIBILITIES Team Leadership/Supervision
  • Oversee the clinical performance and achievement of productivity expectations of Care Team members:
  • Foster an engaging culture among all teammates
  • Manage adherence to established and documented Eventus clinical standards, processes and procedures as written by specialty specific clinical directors. Receive and react to clinical quality data reports
  • Regularly review documentation and coding of all care team members to ensure compliance and quality
  • Receive and analyze clinical quality data and provider productivity reports; reacting and counseling team members, as needed, to maintain standards
  • Handle general practice clinical questions. Collaborate with collaborating physicians, clinical leaders, and/or director of clinical services, when specialty specific guidance is needed.
  • Act as clinical and workflow expert for providers within the team and help facilitate the resolution of clinical inquiries from providers, facility personnel, and family members
  • Regularly visit on site with care team members to observe and optimize clinical skills and workflow
  • Partner to create an environment of clear responsibility, authority, autonomy, and accountability that energizes and encourages staff to perform at their personal best:
  • Manage care team members as assigned
  • Ensure providers are performing clinical duties including caring for all assigned patients. Receive reports on provider productivity and react as needed to maintain standards
  • Work to resolve clinical management issues such as attendance, documentation, adherence to clinical policies and procedures, and all clinical duties
  • Collaborate with the care team administration and regional operators to ensure metrics are met
  • Ensure that all patients in your care team receive the services they need.
  • Track the caseloads in each building and monitor for adherence to best practices for medically necessary visits
  • Work to assign providers to facilities/patients to be sure that all services are covered for each building based on those contracted
  • Monitor for any absences in coverage for reasons such as PTO, resignations, relocations, or territory realignment and ensure other coverage is found
  • Perform clinical duties to ensure that gaps in coverage do not exist
  • Assist in growth of the team through hiring and recruiting:
  • Monitor territory of providers, work with CTC to track monthly all of a providers’ territories, caseload, and schedule
  • Be aware of any providers with modeled capacity or interest in additional work
  • Work with business development on starting new buildings promptly
  • Assist with interview process and coordination of remote and field interview with clinical experts for the open position
  • Assist with non-specialty specific education and training and onboarding of new providers
  • Maintain a customer service focus and assist with problem-solving of facility issues:
  • Maintain active knowledge of leadership at all team facilities
  • Visit all facilities in team quarterly
  • Be available as first call from facilities for any concerns they are having. Work between the facility and your clinical team or Eventus internal support to determine a resolution. Communicate resolution back to the facility.
Care Team Clinical Integration
  • Ensure team cohesion through communication, setting expectations, and leading regular team meetings
