Position Overview:Day Service Specialist
As a Day Service Specialist, you’ll develop and implement community-based, person-centered day service plans. You’ll build strong relationships with the individuals you support, while also engaging with their teams and the broader community.
What You’ll Do:- Plan and coordinate individualized community activities to promote independence and social interaction.
- Provide direct support to clients, including personal care, medication administration, and use of adaptive equipment.
- Document daily progress and services provided.
- Transport clients using a company‑provided vehicle.
- Engage in community outreach and utilize technology to support your work.
- Work independently and collaboratively with a supportive team.

- Must live within 15 miles of the service area (Shakopee)
- Valid driver’s license and space for overnight parking of company vehicle
- Home workspace with reliable internet and privacy
- Must pass a Department of Human Services background study
