Full-Time Registered Nurse Home Health Visits – $10k Sign‑On Bonus

$10k Sign‑On Bonus available for full‑time positions.

Location: Northumberland County, PA – services adult and geriatric clients on a per‑visit basis.

• Follow a designated care plan in accordance with patient needs
• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments and procedures as assigned
• Monitor clients’ conditions and report changes to Clinical or Client Services Manager
• Execute and document doctors’ orders accurately
• Perform client assessments as necessary
• Case management and care coordination
• Document observations, interventions and evaluations using the touch‑pad tablet system

• Current license as a Registered Nurse in Pennsylvania
• Minimum of one year recent, verifiable clinical (medical/surgical) experience
• Prior home care experience strongly preferred, but not required
• Graduation from an accredited nursing program (transcript or diploma proof)
• Ability to work independently and manage time effectively
• Strong interpersonal skills
• Solid computer skills; electronic medical record experience preferred
• Willingness to travel to cases as assigned
