About Community Horizons:
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children s Long-Term Support (CLTS) program. Self-direction allows participants to make their own choices selecting who provides their care and how those services are delivered.
