Job Summary:

Under direct supervision, the CPS Ongoing Case Manager prepares studies of individuals and families in need of social services, makes recommendations regarding the range of services needed, maintains case records and plans appropriate client services in conjunction with the supervisor.

Essential Duties & Responsibilities:

  • Accept requests and referrals for services and determine through interviews, home visits and collateral investigations the range of services needed.
  • Interview clients, evaluate intake information and determine client needs.
  • Explain the scope of services offered and discuss the client's rights and responsibilities in relation to his/her use of such services.
  • Assist in planning services appropriate to client's problems, maintain client contact and review and modify plans as necessary.
  • Prepare studies of individuals and families in need of services and recommend an appropriate course of action.
  • Maintain accurate and current case records, reports and other types of correspondence.
  • Attend in‑service training and staff development activities.
  • May speak before community groups on specific social problems and services.
  • Perform other related duties as required or assigned.

Work Requirements:

  • Bachelor’s of Social Work degree (BSW) from a four‑year college or university, preferred;
  • or a Bachelor’s Degree and one to two years of related experience and/or training; or an equivalent combination of education and experience that will allow gaining a state of Wisconsin Social Work License within two years of employment.
  • One to two years of related experience and/or training; or an equivalent combination of education and experience.
