Care Management Nurse
Position Summary / Career Interest
The Care Management Nurse provides intervention for patients enrolled in the care management program and develops comprehensive plans of care for each patient's individual needs that transition across the health care delivery system.
Responsibilities & Essential Job Functions- Accurately gathers and interprets information obtained to identify patients suitable for effective intensive care management intervention.
- Builds relationships with patients and uses assessment skills to identify physical, psycho-social, financial, and environmental health care needs.
- Assesses and identifies appropriate resource utilization, level of care, and treatment options to develop a comprehensive plan of care specific to each member’s needs, aiming to optimize health status, self‑management, and safe transitions.
- Educates patients and ancillary personnel about health care needs, safety issues, and benefits, utilizing patient education tools.
- Collaborates with clinical staff to develop and execute plans of care and achieve goals.
- Researches, secures, and links resources, services, and supports for patients and families.
- Coordinates care, focusing on transitions across the health care delivery system.
- Utilizes electronic tools (e.g., EMR) to document assessments, clinical findings, and progress toward goals.
- Participates in safety initiatives and risk‑mitigating measures, completing all safety competencies and requirements on time.
- Performs professional, clinical, and technical competencies of the assigned unit or department.
- Other duties may be assigned as needed.
Education and Experience
- High School Graduate
- Bachelor’s Degree in Nursing
- Licensed Registered Nurse (LRN) – Multi‑State. Must hold an active multistate RN license in good standing in the state of residence or a single‑state RN license in the state of employment, with proof of application for a multistate license if required.
- Staff hired before 2/1/2024 must obtain a multistate RN license from the state of residence by the next renewal by 3/31/2026.
- Advanced Cardiac Life Support (ACLS) – American Heart Association (AHA)
- Basic Life Support Heartsaver (BLS HS) – American Heart Association (AHA)
- Strong computer skills
- Efficient entry of information into EMR while interacting with patients telephonically or in person.
