• Ensure all activities align with Elara Caring’s vision and hospice policies.
• Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
• Collect data on factors influencing patient/family/caregiver health status.
• Communicate significant findings, problems, and changes to the Clinical Supervisor, physicians, and other personnel involved in care.
• Provide counseling, instruction, and support to patients, facilities, and families to follow the Interdisciplinary Plan of Care.
• Educate staff in alternate care settings on hospice philosophy and symptom management.
• Offer support at the time of death and conduct bereavement assessment.

• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• Current state license as a Registered Nurse (RN).
• Minimum 1 year of clinical experience; hospice or home‑health experience preferred.
• 50% travel required.
• Physical ability to sit, stand, bend, lift, and move intermittently and lift 50–100 lbs.
• Valid driver’s license, insurance, and reliable transportation.

You will report to the Clinical Team Manager.
