- Ensure all activities align with Elara Caring’s vision and hospice policies.
- Assess hospice patients and families to identify physical, psychosocial, and environmental needs.
- Collect data on factors influencing patient/family/caregiver health status.
- Communicate significant findings, problems, and changes to the Clinical Supervisor, physicians, and other personnel involved in care.
- Provide counseling, instruction, and support to patients, facilities, and families to follow the Interdisciplinary Plan of Care.
- Educate staff in alternate care settings on hospice philosophy and symptom management.
- Offer support at the time of death and conduct bereavement assessment.
- Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
- Current state license as a Registered Nurse (RN).
- Minimum 1 year of clinical experience; hospice or home‑health experience preferred.
- 50% travel required.
- Physical ability to sit, stand, bend, lift, and move intermittently and lift 50–100 lbs.
- Valid driver’s license, insurance, and reliable transportation.
You will report to the Clinical Team Manager.
