PM Nurse Supervisor; LPN or RN

Pine Haven has an exciting opportunity for an LPN or RN looking to make a difference. The PM Nurse Supervisor at Pine Haven Christian Home will supervise the day‑to‑day activities performed by CNAs/RAs, provide direct nursing care to residents, and monitor their overall well‑being at our Haven Drive campus.

• Lead and direct the day‑to‑day functions of floor staff on the unit in accordance with current laws, rules, and regulations governing our facilities.
• Ensure that all nursing personnel comply with Pine Haven’s written policies and procedures.
• Ensure that sufficient staffing is maintained to provide quality care that meets the daily needs of residents; develop work assignments and monitor staff absenteeism, reporting issues to the unit manager.
• Make shift rounds to observe the physical and psychosocial status of residents.
• Provide direct nursing care as necessary, administer prescribed medications and treatments per MD orders, and maintain competence in all required skills within the facility.
• Attend team meetings as scheduled.
• Ensure that residents are treated with respect and dignity, performing nursing duties with the comfort and well‑being of each resident in mind.
• Participate in weekend on‑call rotation with Assisted Living Managers and other administrative clinical staff members.
• Work beyond normal work hours and in other positions temporarily when required; may be subject to call‑back under emergency circumstances.
• Immediately report any suspected abuse or neglect per facility policy.
• Maintain a clean, safe, and efficient work environment.
• Report any observed changes in a resident’s needs and/or condition immediately and as appropriate.

• Nursing degree from an accredited college or university, or graduate of an approved LPN or RN program.
• An unencumbered, active license to practice as a Licensed Practical Nurse or a Registered Nurse in the State of Wisconsin.
• Minimum three years’ experience as a staff nurse in a hospital, long‑term care facility, or other related health care facility. Supervisory experience preferred.
• Demonstrated leadership/supervisory skill and willingness to work harmoniously with others; ability to make independent decisions when circumstances warrant such action.
• Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to long‑term care.
