, a skilled nursing facility in the Eden Senior Care Network, is now hiring a Physical Therapist to provide rehabilitation services in one or more of our service locations!
Position: Full-Time Physical Therapist

Responsibilities- Develop and implement individualized therapy programs designed to restore, reinforce and enhance performance
- Identify need for and make referral to other disciplines to address the comprehensive needs of the patient
- Instruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathology
- Supervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards
- Develop care plans using a variety of treatment techniques
- Create fitness- and wellness-oriented programs tailored to patients’ specific needs
- Provide quality, personalized and evidence-based care and proven interventions
- Motivate patients during treatment in order to help them function optimally
- Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
- Evaluate effects and monitor and communicate progress
- Proven work experience as a physical therapist
- Excellent interpersonal communication skills
- Valid license to practice physical therapy
- Graduate degree or greater in physical therapy
