Physical Therapist Sheboygan Senior Community – Sheboygan, WI Pay Rate: $52–$55 per hour

Schedule:

PRN | Approximately 6–10 hours per week

Sheboygan Senior Community (SSC) has been proudly serving seniors in Sheboygan County for over 60 years. We provide a full continuum of care—including assisted living, skilled nursing, short-term rehabilitation, respite care, and end-of-life services. Our resident-centered philosophy and dedicated therapy team ensure that each individual receives compassionate, high-quality care.

We are currently seeking a Physical Therapist to join our therapy department. This position offers flexibility, a supportive work environment, and the opportunity to make meaningful impacts on the lives of our residents.

• Provide high-quality physical therapy evaluations and treatments to residents in skilled nursing and rehabilitation settings
• Develop individualized treatment plans to support mobility, function, and overall well-being
• Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and other care staff
• Document services in accordance with regulatory and facility standards
• Educate residents and families on therapy goals, progress, and home exercise programs
• Maintain professionalism, empathy, and a resident-first approach at all times

• Current Wisconsin Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Previous experience in a skilled nursing or rehab setting preferred, but not required
• Strong communication, time management, and clinical skills
