We are currently seeking a PART-TIME Therapy Clinical Services Manager for our Northern Home Health Therapy Division. This is a part-time position located in our Appleton, Fond du Lac, or Sheboygan office.
Responsibilities- As a member of our leadership team, develop trusting relationships with employees, patients, families, and the community.

- Provide incredible team support.

- As the Regional Therapy Manager, assume responsibility for the review of the Plan of Care for home health patients and make necessary changes to maintain organization, state, and federal compliance. This includes:
- Timely processing of day-to-day workflow tasks with knowledge of CMS and value-based purchasing guidelines.
- Review incoming referrals to determine home care eligibility.
- Collaborate with the therapy scheduler to ensure successful hand-off services.
- Host case conferences for the northern counties.
- Lead discipline-specific meetings.
- Conduct annual reviews, competency training, onsite audits, and triage cases with the active therapy team.
Other responsibilities include, but are not limited to:
- Processing orders.
- Analyzing data and clinical metrics.
- Assisting with DME handling.
- Chart audits.
- Intake obligations.
- Working with the quality team.
The manager will have administrator on-call requirements approximately once every 8-10 weeks.
Qualifications- Sound therapy skills and experience.
- At least one year of experience in home care.
- Active credentials in the state of Wisconsin.
- Ability to work independently.
- Excellent written and oral communication.
- Exceptional time management skills.
