Assisted Living Nurse PT

Overview

Sheboygan Senior Community is seeking a compassionate and dedicated part-time nurse to join our Assisted Living unit. Our CBRF Assisted Living facility features 25 single-bedroom units and caters to a predominantly independent population. Notably, our facility operates as a no-mechanical lift environment, and is not a locked memory care unit. We are seeking individuals who are dedicated to person-centered care and are eager to make a meaningful impact in the lives of seniors every day.

Pay & Shift Details:

• Pay based on experience and skills
• Shift Details: two days a week, 4-hour shifts (days and times are flexible)
• Additional hours available upon request

• Process physician orders efficiently and accurately.
• Review and update residents' care plans regularly.
• Monitor and evaluate resident weights and vital signs, providing updates to the Manager and Physician as necessary.
• Serve as the primary clinical contact for resident medical concerns while on-site.
• Assist in the development and delivery of staff clinical education programs.
• Conduct thorough reviews of resident charts to ensure compliance with assessment standards.

• Phlebotomy (blood draw) training a plus
• LPN license in the State of Wisconsin
• Experience in long-term care or geriatric nursing preferred
• Excellent communication and critical thinking skills
