- As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for your home health patients making necessary changes to maintain quality of care delivered. This includes:
- Making the initial nursing evaluation and creating an appropriate and patient-specific plan of care
- Identifying the patient's/family's physical, psychosocial, and environmental needs and tailoring each care plan appropriately
- Completing recertifications or resuming care for eligible patients
- Initiating and coordinating the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition
- Documenting problems, appropriate goals, interventions, and patient/family response to skilled nursing care
- Collaborating with the patient/family, attending physician, and other members of the IDG in providing patient and family care
- Experience Requirements: One year of clinical experience preferred.
- License Requirements: Current Wisconsin RN licensure
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle
- Location: Sheboygan and Manitowoc Counties
- Schedule: Full-Time, 1st shift Monday-Friday. Minimal on-call required.
- Compensation: Negotiable based on experience.
- Sign-On Bonus: Up to $7,500 available, depending on location and experience.
