As a Band 5 Community Nurse, you will join our valued Community Nursing Team based at Sheppey Community Hospital, providing care across Sheppey, Sittingbourne and the surrounding villages. As a Community Nurse, you will provide high quality nursing care to patients within their own homes and community settings. You will assess, plan, implement and evaluate care, ensuring patients receive safe, effective and compassionate support throughout their healthcare journey. Working both independently and as part of a wider multidisciplinary team, you will build strong relationships with patients, families and carers, helping them manage their health and wellbeing while promoting independence wherever possible.
You will have the opportunity to utilize and develop a broad range of clinical skills while managing a varied caseload. This role offers autonomy, variety and the chance to make a meaningful impact within your local community every day.
- Deliver safe, effective and person-centered nursing care in community settings
- Assess patient needs and contribute to care planning and review
- Support patients with acute, chronic and complex health needs
- Collaborate with GPs, therapists, social care teams and other healthcare professionals
- Promote patient independence, health and wellbeing
- Maintain accurate and timely clinical documentation
- Contribute to service improvement and share best practice across the team
A full UK driving licence and access to a vehicle for work purposes is essential.
Person Specification- Registered Nurse with a valid NMC registration
- Passion for delivering high quality, patient-centered care
- Strong communication and interpersonal skills
- Good organisational skills and the ability to manage a varied workload
- Ability to work independently and as part of a multidisciplinary team
- Flexible and adaptable approach to work
- Confidence using IT systems, including Microsoft Office applications
